Cycle 3: Integrated Care for Sustainable Health Systems: Highlighting Innovative Approaches
Fall 2025: October 21–December 2, 2025
This ECHO cycle explored how integrated models of care—particularly those that bridge clinical services with social supports—can advance both human and planetary health. Strategies such as social prescribing, improved access to primary care providers, co-locating providers, and care coordination help address the social determinants of health. By shifting care upstream and connecting patients with appropriate medical services and community-based resources, these approaches improve health outcomes and reduce reliance on resource-intensive, hospital-based care—contributing to a lower carbon footprint in healthcare delivery. Additionally, by fostering stronger social infrastructure and more equitable access to care, these systems build climate-resilient communities—ones better equipped to withstand and recover from climate-related shocks like extreme heat and wildfires.
With a particular focus on social prescribing, this cycle highlighted how integrated care models promote environmental sustainability by reducing unnecessary interventions, transportation, and waste, while simultaneously improving population health outcomes and climate resilience. Through short lectures and case presentations, we considered the many ways in which strengthening the connection between medical care and social support can lead to a healthier, safer, more sustainable future for both people and the planet.
This cycle was done in partnership with the Canadian Institute for Social Prescribing (CISP).

Cycle 2: Resource Stewardship for High-Quality Care
Spring 2025: April 8–May 27, 2025
The ability to deliver high-quality care and extend the provision of health services to future generations requires careful stewardship of resources and consideration of the broader impacts of healthcare delivery on the environment, and the determinants of health. When care is provided, we must ensure it is appropriate to avoid harmful overuse and underuse. And when appropriate care is provided, we must ensure it is as sustainable as possible to enhance the health sector’s climate resilience and reduce its contributions to climate change and other environmental problems. These efforts are deeply tied to achieving quality co-benefits, such as improving patient outcomes and reducing costs. In this ECHO cycle, we will explore resource stewardship as a key component in the delivery of high-quality, truly sustainable care. We will consider the following two themes:
- RETHINK: Considering practice and operational changes that reduce the use of unnecessary products and services
- REPLACE: Considering more sustainable alternatives to environmentally impactful products and services

Cycle 1: Pilot cycle on Climate, Health & Sustainable Care
Fall 2024: October 8–December 10, 2024
Within the Canadian health sector, there is growing recognition of the impacts of climate change on health and health systems, and the many ways in which the health sector itself is contributing to climate change and other environmental problems. Emerging sustainability champions have been calling for change as they initiate climate adaptation, resilience, and mitigation projects at all levels of the health system. As our collective understanding of the issues and opportunities related to climate change deepens, members of the healthcare community need support as they navigate the nuances of sustainability work.
In this ECHO cycle, we will explore two overarching themes:
- Confronting acute and chronic climate risks: Explores climate challenges for health and health systems, including specific health risks and the range of shocks and stresses to which health systems must adapt.
- Implementing climate mitigation and adaptation in practice: Explores a range of healthcare practices and identifies the specific adaptation and mitigation measures arising in each.
